Effective dose management in mobile C-arms is essential for balancing patient safety with premium image quality, especially in today’s surgical environments. Ziehm Imaging stands out with its SmartDose1 concept, a comprehensive suite of intelligent features designed to automatically optimize dose levels without sacrificing image quality or workflow efficiency. By integrating advanced hardware and software solutions, SmartDose is adapting to clinical needs in real time. This holistic approach combines elements like optimized pre-filtration of the X-ray beam, adaptive pulsed fluoroscopy, and real-time object detection to deliver premium performance across a wide range of procedures, while adhering to ALARA (As Low As Reasonably Achievable) principles. Ziehm Imaging’s new whitepaper provides a deeper look into this concept, explains the underlying mechanisms and offers a detailed examination of how effective dose management can be achieved in practice. Key components of SmartDose SmartDose encompasses multiple features that work in tandem to reduce dose. Combined, these features provide a seamless workflow experience that prioritizes image quality, patient safety and dose efficiency. LASER POSITIONING DEVICE Allows for precise patient alignment without unnecessary exposure. OBJECT DETECTED DOSE CONTROL (ODDC) Detects patient anatomy in real time and dynamically adjusts exposure parameters and noise reduction filters, while also featuring an automatic pulse rate reduction during static imaging for optimal dose and image quality. HIGH-SPEED AUTOMATIC DOSE REGULATION (ADR) High speed X-ray exposure control for optimal exposure parameters, including tube voltage and tube current, ensuring excellent image quality at minimized dose levels. ZAIP (ZIEHM ADAPTIVE IMAGE PROCESSING) Advanced image processing algorithms and noise reduction filters that enhance contrast between soft tissue, bone, and metal to visualize even finest structures like guidewires. VIRTUAL COLLIMATORS For exposure-free positioning of slot and iris collimators. BEAM FILTRATION BeamFiltration2 is an advanced anti-scatter grid paired with an optimized beam hardening which selectively removes low-energy X-ray photons, thereby significantly reducing patient dose and improving image quality. REMOVABLE ANTI-SCATTER GRID Enhances image contrast by suppressing scattered radiation. For pediatric or low-dose applications, the grid can be removed to reduce entrance skin dose. PREMAG Digital pre-magnification of the live monitor image when switching to a different magnification mode, allowing the user to preview the field of view of the next image taken without additional exposure. Additional considerations In addition to advanced X-ray beam filtration, SmartDose leverages pulsed fluoroscopy to further minimize dose levels. Unlike continuous fluoroscopy, which delivers a constant beam, pulsed operation emits short, high-frequency bursts of X-rays. Ziehm Imaging’s monoblock generators produce well-defined pulses with minimal overlap and heat generation, enabling a significant dose reduction compared to continuous modes (Figure 1). To optimize both image quality and dose efficiency, the system dynamically adjusts pulse rates: higher frequencies (e.g. 25 pps) are used during motion to avoid blur, while lower rates (e.g. 8 pps) are applied when anatomy is at rest. High-speed ADR ensures seamless transitions, while adaptive noise reduction filters are applied in real time to maintain image clarity at reduced dose levels. Conclusion Ziehm Imaging’s SmartDose represents a paradigm shift in dose management for mobile C-arms. This technology not only upholds ALARA principles but also enhances clinical reliability and image quality. SmartDose empowers healthcare professionals to achieve superior outcomes with minimal health risk to patients, setting a new standard in interventional imaging.

Ziehm Imaging continues to shape the future of mobile imaging, integrating increasingly sophisticated algorithms into its C‑arms to support optimal clinical performance. In the recently published whitepaper “Image Processing Redefined”, Ziehm Imaging outlines current developments in fluoroscopic image processing and their relevance for modern interventional workflows. As procedural precision demands continue to rise, imaging chains must evolve accordingly, making advances in detector and generator technology essential to maintain consistent image quality. In this context, the whitepaper details the first in the market true 2k2k image processing chain QuantumStream and its role in achieving stable high‑resolution fluoroscopy. Image processing as a cornerstone Fluoroscopic image processing has evolved into a key enabler of image quality in modern X-ray systems. Covering the entire imaging chain from acquisition to display, it combines dose management, noise reduction, contrast optimization, and artifact suppression to ensure high-quality images at minimal dose levels. This is particularly important in minimally invasive interventions, where image clarity and efficiency directly influence clinical outcomes. The role of anatomical programs (APRs) APRs are preset imaging programs tailored to specific body regions and clinical applications. By automatically adjusting imaging parameters to anatomical density and motion, they simplify operation and support consistent, high-quality imaging across various procedures. The importance of Object Detected Dose Control (ODDC) Compensating for patient movement is essential to keep dose levels low while maintaining image quality. ODDC analyzes anatomy in real time and adapts both image processing and pulse rates accordingly. When the patient is still, the system applies maximum noise reduction and lowers pulse frequency to minimize dose without compromising clarity. During movement, however, ODDC increases pulse rates and reduces filtering to avoid blur. This intelligent adjustment ensures smooth fluoroscopy, consistent image quality, and optimized dose efficiency throughout the procedure. The mechanical depth of the C-arm is defined as the distance from the base of the C to the midpoint of the free space between X-ray source and detector. For Ziehm Imaging’s premium systems, this value is 68cm. With our 31cm × 31cm detectors, half of the detector size extends beyond this axis. Thanks to advanced image processing such as ODDC, in combination with asymmetric collimators, a diagnostic image quality can be maintained across the full detector area, making this additional range fully usable. In practice, this results in an effective C-arm depth of up to 83cm – providing clinicians with more working space and flexibility during procedures. Additionally, Ziehm Imaging C-arms feature a compact vertical extension of 42 cm, compared to the 45–50 cm offered by conventional systems. This design incorporates an ultra-flat detector, integrated within the C-arm, which minimizes downward protrusion thus increasing the effective floor-to-detector height, which is similar to competitors’ systems at maximum extension. The technology behind QuantumStream QuantumStream combines premium CMOS technology with a unique 100 µm pixel pitch and full 2k2k image processing – displayed in razorsharp detail on a native 4k monitor. This delivers an unmatched spatial resolution of up to 5.0 lp/mm, setting a new standard for mobile fluoroscopy. With advanced contrast optimization, Quantum-Stream excels in demanding applications: from cardiovascular imaging, where subtle vessel details must remain visible, to spine procedures with strong density variations, where contrast stabilization prevents loss of information. Supported by a unique, compact and powerful 30 kW generator, the system achieves these performance gains without increase in patient dose – combining precision with patient safety. Ziehm Adaptive Image Processing (ZAIP) ZAIP features advanced real-time algorithms designed to deliver optimal fluoroscopic image quality. A spatial, edge-preserving noise reduction filter suppresses background noise while maintaining fine anatomical detail. In parallel, an intelligent motion-sensitive temporal filter adapts to patient or instrument movement, reducing noise effectively without introducing blur. Complementing these noise controls, frequency-based contrast stabilization selectively enhances clinically relevant structures while preventing oversaturation in areas of varying density. Together, these technologies ensure consistently clear and detailed visualization – from guidewires and catheters to complex anatomical regions – even under low-dose conditions. Ziehm Iterative Reconstruction (ZIR) for 3D systems ZIR brings iterative reconstruction techniques – previously limited to CT scanners – to Ziehm Imaging’s mobile 3D C-arms, automatically decreasing fan-beam and metal artifacts, resulting in a more distinguishable anatomy and easily defined bone crests. This, in turn, enhances volume rendering greatly, without increasing dose, which is especially important for complex orthopedic and vascular procedures. Conclusion With technologies such as QuantumStream, ODDC, tailored APRs, ZAIP and ZIR, Ziehm Imaging brings premium image processing to mobile C-arms. This advanced imaging chain unlocks the full potential of high-resolution CMOS detectors while maintaining a clear focus on patient safety. Compensating for patient movement is essential to keep dose levels low while maintaining image quality.

The technology behind QuantumStream
QuantumStream combines premium CMOS technology with a unique 100 µm pixel pitch and full 2k2k image processing – displayed in razorsharp detail on a native 4k monitor.

Ziehm Adaptive Image Processing (ZAIP)
ZAIP features advanced real-time algorithms designed to deliver optimal fluoroscopic image quality.

Ziehm Iterative Reconstruction (ZIR) for 3D systems
ZIR brings iterative reconstruction techniques – previously limited to CT scanners – to Ziehm Imaging’s mobile 3D C-arms, automatically decreasing fan-beam and metal artifacts, resulting in a more distinguishable anatomy and easily defined bone crests. This, in turn, enhances volume rendering greatly, without increasing dose, which is especially important for complex orthopedic and vascular procedures.

Conclusion
With technologies such as QuantumStream, ODDC, tailored APRs, ZAIP and ZIR, Ziehm Imaging brings premium image processing to mobile C-arms. This advanced imaging chain unlocks the full potential of high-resolution CMOS detectors while maintaining a clear focus on patient safety.

Ziehm Imaging presents its latest innovations Endovascular Navigation1 Peripheral2 and PositionPilot2 to the public for the first time at this year’s European Congress of Radiology (ECR 2026). After having released Endovascular Navigation Aorto-iliac last year, Ziehm Imaging now introduces its endovascular navigation software designed for peripheral artery disease conditions. The innovative solution is integrated into the mobile C-arm and allows for extensive panorama creation as well as navigation of the lower limb. Furthermore, the new PositionPilot allows for straightforward control of the C-arm and the compatible OR table via a single user interface – directly from the sterile field. Visitors can also get an impression of the numerous new features now available for the Ziehm Vision RFD 3D. Navigate with ease and optimal overview Endovascular Navigation Peripheral allows for panorama creation for peripheral artery disease cases by stitching fluoroscopic and angiographic images into a complete, up-to-date overview. Thrombotic and stenotic areas can be easily identified and marked. “Live fluoroscopic images align precisely with the angiographic panorama through image fusion—allowing clinicians to navigate with ease and optimal orientation without additional contrast media,” says Martin Törnvik, Vice President Global Sales & Marketing. Artificial intelligence-based algorithms automatically match and display image fusion – adapting in real time to patient, table, or C-arm movement. The solution is fully integrated into Ziehm Vision RFD Hybrid Edition3 (Cardio) and Ziehm Vision RFD 3D. Closing the gap to fixed installed systems with PositionPilot Motorized OR tables are standard in fixed installations – Ziehm Imaging transfers this logic into the mobile world. The innovative PositionPilot allows for straightforward handling of the mobile C-arm and OR table via the intuitive Position Control Center plus. With PositionPilot, the surgeon has everything at hand and can operate both devices easily from the sterile field. It enables fast, reproducible, and reliable intraoperative positioning. Two joysticks allow for easy control of all four axes of the C-arm, while table height as well as horizontal and vertical sliding are controlled with a separate joystick. For smoother workflows, PositionPilot facilitates the recall of anatomical positions: Up to three synchronized C-arm and OR table positions plus one additional table position can be recalled at any time during the procedure. Exceptional clarity at minimum dose with Ziehm Vision RFD 3D With several major innovations, Ziehm Imaging further strengthens intraoperative 3D imaging and dose management with Ziehm Vision RFD 3D. The new Adult HQ dose mode complements the existing Low Dose, Adult, and Obese Patient settings, providing the high penetration and resolution needed for demanding cases such as cervical–thoracic junction imaging, complex tumors, and hip or lumbar procedures. The new 25 cm volume3,4 significantly expands anatomical coverage, enabling visualization e. g. of the entire pelvis. A dedicated anatomical program for bronchoscopy now supports optimal 2D and 3D visualization of fine lung structures and updated settings enhance cochlear implant imaging. In addition, the new surface rendering option delivers detailed 3D views for implant and position control, particularly valuable in maxillofacial surgery. High-quality mobile imaging from head to toe On the joint booth with its daughter companies Orthoscan and Therenva, Ziehm Imaging will be displaying further mobile imaging solutions. All Ziehm Imaging C-arms featured onsite are part of the premium portfolio CMOSline5 that is elevated with QuantumStream, the first-to-the-market true 2k2k imaging chain. The premium portfolio comes with up to 30 kW generator power (according to IEC 60601-2-54) and a 100 µm pixel size CMOS detector. From general cardiac surgery to electrophysiology and high-end mobile cath lab procedures such as mitral clippings and TAVIs, Ziehm Vision RFD Hybrid Edition Cardio is a space-saving and cost-efficient alternative to fixed cath labs. With its all-in-one design, Ziehm Solo FD is one of the most compact C-arms on the market. This product line does not require a separate monitor cart, making it easy to maneuver even in the smallest treatment scenarios.

At this year’s RSNA, Ziehm Imaging is exhibiting solutions for orthopedics as well as a complete mobile hybrid room for cardiovascular applications. Physicians are able to see the first endovascular navigation with image fusion technology integrated into a mobile C-arm: Endovascular Navigation1 Aorto-iliac will be demonstrated live at the booth. Visitors can also expect the latest member of Orthoscan’s mini C-arm product portfolio focusing on extremity imaging: Versa is a light-weight, portable imaging solution for seamless integration into any setting. First endovascular navigation integrated into a mobile C-arm Ziehm Imaging has introduced Endovascular Navigation Aorto-iliac with image fusion technology for the first time earlier this year. At RSNA, visitors can see for themselves how the solution – the first of its kind integrated into a mobile C-arm – combines imaging and vascular navigation on a single user interface. This design improves user-friendliness and saves space in the operating room. The technology, which will be showcased on Ziehm Vision RFD Hybrid Edition2, delivers greater confidence during hybrid procedures while helping to reduce radiation exposure and contrast media usage. Using artificial intelligence-based algorithms, Endovascular Navigation enables fast and easy image fusion. The software is designed for interventions in the aorto-iliac region and can e.g. be used for EVARs as well as for complex interventions such as FEVARs. “We are proud to showcase Endovascular Navigation Aorto-iliac at RSNA. At our booth, physicians can experience firsthand how the dedicated workflow on one single user interface provides greater ease of use,” says Martin Toernvik, Vice President Global Sales & Marketing. Ziehm Vision RFD Hybrid Edition Cardio as a space-saving and cost-efficient alternative to fixed cath labs Ziehm Vision RFD Hybrid Edition is also available with a dedicated cardiology option package to cover the broad range of cardiological applications. From general cardiac surgery to advanced electrophysiology and high-end mobile cath lab procedures such as mitral clippings, TAVIs and TMVRs, Ziehm Vision RFD Hybrid Edition Cardio is a space-saving and cost-efficient alternative to fixed cath labs. With its modular mobile Cath Lab setup, the system allows hospitals to select only the components they truly need – from imaging and table options through advanced monitoring to diverse monitor options. Orthoscan Versa – designed for unparalleled mobility Ziehm Imaging will also feature the newly introduced Orthoscan Versa, a fully portable imaging solution designed to integrate seamlessly into any clinical environment—whether placed on a desktop or mounted on an articulating monitor cart. The intuitive touchscreen monitor offers a tablet-like experience, removing the need for bulky components and helping streamline the clinical workspace. Versa delivers advanced imaging capabilities, including 180° orbital rotation for improved anatomical visualization, selectable pulse fluoroscopy technology, and pediatric imaging approval. With its exceptional portability, Versa enables smooth transitions between satellite clinics and off-site locations, enhancing operational efficiency and elevating patient care. High-quality mobile imaging from head to toe On the joint booth with its daughter companies Orthoscan and Therenva, Ziehm Imaging will be displaying further mobile imaging solutions. All Ziehm Imaging C-arms featured onsite are part of the premium portfolio CMOSline3 that is elevated with QuantumStream – a completely new imaging system. The premium portfolio comes with up to 30 kW generator power (according to IEC 60601-2-54), a 100 µm pixel size CMOS detector and the first-to-the-market true 2k2k imaging chain. Ziehm Vision RFD 3D covers a wide range of applications, from high-end maxillofacial, orthopedics, traumatology and spinal surgery through bronchoscopy procedures to innovative vascular interventions.

ECR 2025 | 26.

At this year’s European Congress of Radiology (ECR 2025), Ziehm Imaging is going to present its latest innovation for vascular surgery for the first time. The new software package, available as part of the mobile hybrid room solution, supports physicians in their endovascular interventions with planning, navigation and fusion imaging. The market launch will take place directly with the presentation at ECR. First clinical evidence has already successfully been realized at a renowned university hospital. First Endovascular Navigation1 with image fusion technology integrated into a mobile C-arm Ziehm Imaging’s hybrid solution based on the Ziehm Vision RFD Hybrid Edition2 has been established successfully in the market for years. For the first time, the function of fusion imaging is integrated into the C-arm, eliminating the need for a separate device. This allows greater ease of use and saves valuable space in the operating room as imaging and intraoperative vascular navigation is now combined into one single user interface. Greater Precision and Reduced Radiation Exposure Intraoperative Endovascular Navigation enables physicians to achieve greater precision in demanding hybrid interventions. The combination of preoperative CT data and intraoperative fluoroscopic images provides even more accurate results while reducing radiation dose and contrast agent use. Using artificial intelligence-based algorithms, vascular image fusion is automated and acts fast. The Endovascular Navigation software is designed for interventions in the aorto-iliac region. It can be used for EVARs as well as for complex interventions such as FEVARs. Prof. Dr. med. Johannes Kalder from University Hospital of Giessen and Marburg reports: “We are very proud to be the first in the world to use this groundbreaking new technology on a mobile C-arm. Endovascular navigation allows us to bring planning and fusion imaging to the C-arm and work directly from the sterile field as we are used to. Image quality and workflow improvements are almost equivalent to those of fixed installed systems. We are particularly impressed by the automatic re-registration after changing the position of the C-arm. The savings in procedure time and expected reduction in contrast agent and radiation dose make a real difference for us and our patients in the further course of care.” A complete new imaging system for Ziehm Imaging’s premium portfolio On the joint booth with its daughter companies Orthoscan and Therenva, Ziehm Imaging will be presenting further solutions in the field of mobile imaging. All Ziehm Imaging C-arms showcased onsite are part of the premium portfolio CMOSline3 that is elevated with QuantumStream – a completely new imaging system. The premium portfolio comes with up to 30 kW generator (according to IEC 60601-2-54), the 100 µm pixel size CMOS detector and the first 2k2k image processing. It also features a 4K UHD monitor. With its all-in-one design, the Ziehm Solo FD is one of the most compact C-arms on the market. This product line does not require a separate monitor cart, making it easy to maneuver even in the smallest treatment scenarios. Ziehm Vision RFD 3D covers a wide range of applications, from high-end orthopedics, traumatology and spinal surgery to innovative vascular procedures. These innovations emphasize Ziehm Imaging’s commitment to advancing mobile imaging technology. The Orthoscan TAU 2020, a mini C-arm for hand and foot surgery, completes the portfolio. It has the largest detector available for a mini C-arm and a 32” 4K resolution diagnostic touchscreen monitor for exceptional viewing.

Visitors to the exhibition stand can also expect solutions for cardiology and vascular surgery as well as other highlights from the product portfolio. Ziehm Solo FD for different requirements with a small footprint With its all-in-one design, the Ziehm Solo FD is one of the most compact C-arms on the market. The product line does not require a separate monitor cart, making it easy to maneuver even in the smallest treatment scenarios. “I am delighted to be able to provide the right solution for different applications with the extended product range for the Ziehm Solo FD,” says Martin Törnvik, Vice President Global Sales & Marketing. The premium version Ziehm Solo FD CMOSline1 is equipped with the latest CMOS detector technology. It supports the detection of the smallest anatomical structures, and ensures excellent image quality with significantly reduced exposure. To enable a wide range of applications, one of the new configurations provides a large 31 cm x 31 cm IGZO flat-panel detector. In orthopedics, for example, the entire hip can be displayed. The company is launching the Ziehm Solo FD lite2,3 to meet the requirements of price-sensitive markets. Complete hybrid solution combined with Therenva’s EndoNaut® system The range of applications for mobile C-arms is also continuing to grow in the cardiovascular segment.
